Version History…


Version 12.0 — a major upgrade with the following new features:

  • Renamed IOKP Practitioner as “IOKP Master” and Forms as “IOKP Forms.”
  • Updated IOKP’s user interface to enhance the:
    • Organization and integration of its components.
    • Overall design with a more subtle appearance.
  • Added the capability of signing IOKP Master’s (like EZcms1500’s) forms before printing with optional SoftSign™ Personal Signature Fonts.
  • Updated and enhanced the User Manual to balance its technical focus with a practical emphasis on strategies for using IOKP in practice.


Version 11.0 — a major upgrade with the following new features:

  • IOKP’s previous three separate editions (PPP, PPC and CCC — identical except for terminology referring to provider, client and service) were integrated into a single edition treating terminology as a user setting
  • Improved, expanded and updated:
    • Internal User Manual
    • IOKP Settings section
  • New forms (tools) added:
    • Exit Evaluation — a tool for the practitioner to assess treatment outcome and effectiveness
    • Personal Journal — a tool for patients / clients to use to connect with feeling, gain insight and track target symptoms and behaviors
  • Various minor interface and form design adjustments


Version 10.0 — a major upgrade that establishes a new direction in the evolution of IOKP while retaining all of the familiar features of Version 9.0. It is the first off–the–shelf / do–it–yourself version. No longer delivered pre–customized with practitioner information, a new IOKP Setup interface makes it easy for practitioners themselves to customize their copies of IOKP Practitioner and Patient (or Client) Forms — and keep them up to date whenever their practice information may change. IOKP Version 10.0 is available:

  • To download directly as two files from the new IOKP download site:
    • IOKP Practitioner and Patient (or Client) Forms file set folder
    • EZcms1500
  • In a choice of:
    • Three separate editions, which differ only terminology referring to provider, service and consumer, as follows:
      • Edition PPP — Psychotherapist / Psychotherapy / Patient
      • Edition PPC — Psychotherapist / Psychotherapy / Client
      • Edition CCC — Counselor / Counseling / Client
    • Seven different colors for IOKP forms’ design accents, such as borders, or without accents
  • Version 10.1 — a service update that addressed several IOKP Practitioner file issues:
    • Fixed interactive form field problems:
      • Removed unused duplicate fields on the business card page for inhouse printing
      • Correctly reset a non–printing information entry area of envelopes not to print
    • Addressed an envelope printing issue involving truncation of return addresses when printing with some printer models by adjusting the page location of these fields
  • Version 10.2 — a service update that introduced the following changes and features:
    • Enhanced the IOKP Setup interface to include user options to:
      • Add design accents to IOKP’s forms in any of seven colors.
      • Include more professional contact information, such as web site, to IOKP’s business cards.
      • Edit insurance claim field information for IOKP’s Treatment Application form.
    • Added a non–printing envelope outline and printing instructions to IOKP’s Envelope form pages and repositioned the address field for better page balance.
    • Numerous minor design tweaks.


Version 9.0 — a major upgrade that included all of the familiar features of Version 8.1 and added a number of new features, including:

  • Ability to save claim information as a data-only file (FDF) with Adobe Reader.
  • New fields added to the Cover Page to permit user customization when saving a copy of the PDF as a template.
  • Navigation bar buttons updated (functionality and cosmetically).
  • The internal User Manual revised:
    • Sections reorganized into a more logical progression (organized by function) and specified in the book marks window.
    • Instructions added for saving FDF with Adobe Reader
    • Instructions added for migrating user–entered information from previous version IOKP templates to IOKP Version 9.
    • Instructions enhanced for saving FDF with Adobe Acrobat.


Version 8.0 — a major upgrade that introduced more new features and enhancements than any previous version:

  • Added a separate Patient (or Client) Forms interactive PDF file that consists of a complete subset of IOKP (renamed as IOKP Practitioner Kit) forms consisting of all patient (client) completion forms (for direct distribution and/or publication on the practitioner’s professional practice web site)
  • Navigation and function tool enhancements:
    • Section bookmarks
    • Uniform (global) Navigation Bar (located at the bottom of each page)
    • A reset button added to the Navigation Bar (for clearing user–entered information)
    • Page Guide Bars with additional information, including forms’ IOKP print ranges
  • Form enhancements:
    • Added the option of a set of matching color accents for stationery and other forms, with a variety of color sets to select from, including a custom color set.
    • All forms, as appropriate, were made interactive (for convenient keyboard completion)
    • Added the option to include the practitioner’s NPI number to appropriate forms.
    • Added limited formatting (rich text) capability to some interactive form fields.
  • Added the capability of saving user–entered information in a separate, smaller FDF file that will open in IOKP (requires Adobe Acrobat)
  • Cosmetic interface enhancements to freshen IOKP’s appearance
  • Version 8.1 — an almost major upgrade provided as a service update that:
    • Added new forms:
      • Process Note for Manual Completion
      • Process Note for Keyboard Completion (Interactive)
      • Treatment Termination Note (Interactive)
    • Expanded several existing forms:
      • Patient (or Client) Contact Information (Interactive) — improved page design
      • Authorization To Release Information — added competency statement to both:
        • From This Office (Interactive)
        • To This Office (Interactive)
      • Intake Evaluation (Interactive) — added:
        • Mental Status Examination checklist
        • Risk Factors checklist and notes
        • DSM–IV multiaxial diagnosis form
      • Treatment Plan (Interactive) — reorganized into three interactive columns (i.e., Problem, Treatment Goals and Procedures)
    • Tweaked several other forms’ designs


Version 7.0 — a major upgrade that retained all of the functionality and flexibility of previous versions — plus more:

  • Added the ability to save copies of IOKP storing user–entered form field information using Adobe Reader 6 or later or Adobe Reader 7 or later.
  • Replaced the “Exit” buttons with “Save form fields” buttons to automate saving copies of IOKP storing user–entered form field information.
  • Updated the internal User Manual.
  • Updated entitlement to a free, single–user–license copy of EZcms1500 from Version 2 to the latest version of EZcms1500.


Version 6.0 — a major upgrade. In response to consumer demand, two new forms were added:

  • HIPAA Notification of Privacy Practices
  • Acknowledgement of Receipt of HIPAA Notification of Privacy Practices


Version 5.0 — a major upgrade that retained all of the functionality and flexibility of previous versions — plus more:

  • IOKP was completely redesigned with more elegant and easier–to–use interface.
  • Automation of:
    • IOKP — with navigation buttons to ease moving around and to check for future IOKP upgrades.
    • IOKP’s forms — the enhancement of IOKP’s forms with interactive form fields introduces the option of entering information into most of its Office & Business Forms and all of its Clinical Record Forms before printing them.
  • Added an internal, comprehensive and informative User Manual.
  • Form design refinements.
  • Other miscellaneous enhancements and tweaks.
  • Version 5.1 — a minor service update that improved and speeded up automatic Billing Statement calculations.
  • Version 5.2 — a minor service update that:
    • Replaced the EZcms1500 Appendix with entitlement to a free, single–user–license copy of the stand–alone version — thus reducing IOKP’s file size, while adding the advantages of the full version of EZcms1500, including its optimized user interface and User Manual.
    • Adopted the Avery 8160 label (1 x 2 5/8 inch, 30 / page) format:
    • Changed return Address Labels to use Avery 8160 labels.
    • Added keyboard–fillable Mailing Labels for use with Avery 8160 labels.


Version 4.0 — a major upgrade:

  • Updated and added forms to enhance IOKP’s utility.
  • Tweaked design of several forms.
  • Introduced additional IOKP options — offering choices of terminology, including reference to consumers as “patients” or “clients” and reference to the provider/service as “psychotherapist/psychotherapy” or “counselor/counseling.” This option was recommended by one of our valued clients who preferred her kit to reflect a “less medical model.”
  • Version 4.1 — a major upgrade that updated the EZcms1500 Appendix from Version 1.1 to the Version 1.2.


Version 3.0 — a major upgrade:

  • Updated and added forms to enhance the kit’s HIPPA compatibility.
  • EZcms1500, our functional enhancement of the United States’ standard Health Insurance Claim Form, CMS–1500 (formerly HCFA–1500), was appended to IOKP as a courtesy to our customers.
    • Version 3.1 — a minor service update that updated the EZcms1500 Appendix from Version 1.0 to the Version 1.1.
    • Version 3.2 — a minor service update that added a “Release of Information to This Office” form, which was recommended by one of our valued clients.


Version 2.0 — a major upgrade:

  • Introduced IOKP in PDF format.
  • Increased IOKP’s flexibility and functionality:
    • Forms could be printed directly from an office computer on an as–needed basis.
    • Hyperlinks were introduced to ease navigation.
    • Additional forms were introduced.


Version 1.0 — initial release of IOKP as a printed set of affordable, customized office forms for psychotherapists in private practice.
