Version History…


Version 7.0 — a major upgrade that:

  • Updated EZcms1500’s user interface to enhance its:
    • Organization and integration of its components.
    • Overall design with a more subtle appearance.
  • Added the capability of signing EZcms1500’s forms before printing with SoftSign™ Personal Signature Fonts.
  • Added a new form — CMS–1500 Enclosure Note.
  • Updated and enhanced the User Manual to balance its technical focus with a practical emphasis on strategies for using EZcms1500 in practice.
  • Version 7.1 — a minor service update that adjusted the location of some form fields on the CMS–1500 (08/05) form for greater precision.


Version 6.0 — a major upgrade that retained all of the familiar features of Version 5. Completely redesigned and restructured, EZcms1500 Version 6.0 updated the internal User Manual and added greatly expanded functionality and a broader range of features, including systems for:

  • Setting up patient accounts and tracking account claim and billing activity.
  • Preparing and printing direct patient account billing statements.
  • Preparing and printing envelopes for CMS–1500 claim forms and patient billing statements.
  • Creating and identifying EZcms1500 templates containing user–entered information specific to the account’s:
    • Practitioner.
    • Patient.
    • Insurance company.
  • Version 6.1 — a minor service update that enhanced the EZcms1500 user interface.
  • Version 6.2 — a service update that:
    • Corrected calculation errors on the Insurance Claim Record form
    • Correctly reset a non–printing information entry area of envelopes not to print
    • Addressed an envelope printing issue involving truncation of return addresses when printing with some printer models by adjusting the page location of these fields
  • Version 6.3 — a service update that:
    • Repositioned EZcms1500’s Envelope address field for better page balance
    • Updated EZcms1500’s internal User Manual


Version 5.0 — a major upgrade that included all of the familiar features of Version 4 and adds a number of new features, including:

  • Ability to save claim information as a data-only file (FDF) with Adobe Reader.
  • New fields added to the Cover Page to permit user customization when saving a copy of the PDF as a template.
  • An optional code specification field was added for CMS–1500 item 21 (for example, for mental health practitioners to specify “DSM–IV” when payers require use and acknowledgment of that coding system).
  • CMS–1500 (12/90) item 33 field locations adjusted to ease direct entry.
  • Navigation bar buttons updated (functionality and cosmetically).
  • The internal User Manual revised:
    • Sections reorganized into a more logical progression (organized by function) and specified in the book marks window.
    • Instructions added for saving FDF with Adobe Reader
    • Instructions added for migrating claim information from previous version EZcms1500 templates to EZcms1500 Version 5.
    • Instructions enhanced for saving FDF with Adobe Acrobat.
  • Version 5.1 — a minor service update that:
    • Corrected a typo in the bookmarks window.
    • Addressed an EZ–Enter navigation (tab order) issue reported by one user.
    • Added directions to the User Manual for configuring the bookmarks window.


Version 4.0 — a major upgrade, completely rewritten to the standards of the latest revision of the CMS–1500 form that is required after March, 2007). EZcms1500 Version 4 included all of the familiar features of Version 3, except printing copies of the CMS–1500 form itself (all new CMS–1500 submissions must be printed onto the new preprinted “red” forms). EZcms1500 Version 4 added a number of new features, including:

  • A completely updated and expanded internal User Manual with a new Instructions section to get up and running quickly and a new Help section.
  • Additional buttons to further enhance EZcms1500 navigation and operation — clicking them to:
    • Go to EZ–Enter
    • Go to CMS–1500 Form
    • Reset Form
    • Save a Copy
    • Print Form
  • EZ–Enter interactive field labels corresponding to CMS–1500 item numbers for easy reference.
  • When using Adobe Acrobat (Standard or Professional), the ability to save user–entered health claim information, to a separate, smaller “form data file” (FDF format) that:
    • Is linked to EZcms1500
    • Will open EZcms1500 in Adobe Acrobat when double–clicked
  • Version 4.1 — a minor service update that introduced some cosmetic and user interface enhancements.
  • Version 4.2 — a minor service update that introduced the following enhancements:
    • EZ–Enter interactive text fields were underlined to enhance user recognition
    • All page guides and navigation bars were revised cosmetically
    • The navigation buttons were revised cosmetically and functionally
    • The internal User Manual was updated
  • Version 4.3 — a service update (almost a major upgrade) that introduced the following enhancements:
    • Support for completing, printing and saving old version CMS–1500 (12/90) forms, similar to EZcms1500 Version 3.0, was added because:
      • Some non–Medicare payers were still allowing providers may deplete their supplies by using them
      • The official cutoff date for requiring the new CMS–1500 (08/05) forms had been extended until July 1, 2007 due to problems with some printed copies provided by the Government Printing Office and some third party suppliers
    • Updated the internal User Manual.
    • Corrected some typing errors found in EZcms1500 Version 4.2.
