Automate CMS–1500 form preparation: Click, type and print CMS–1500 (“old” 12/90 or “new” 08/05) forms — plus much more (please read on).

The current EZcms1500 version is 7. The latest update is 7.2.

What is EZcms1500?

EZcms1500 is Queen City Computer Press, Inc.’s traditional enhancement of the CMS–1500 (formerly, HCFA-1500) form, the United States’ standard Health Insurance Claim Form that is used to expedite Medicare, Medicaid and private insurance company benefits. EZcms1500 Version 7.2 is a comprehensive, automated, “click, type, print and save” PDF–based software package of integrated, easy–to–use forms for:

  • Preparing, saving and printing:
    • Insurance Claims: CMS–1500 forms (both 12/90 and 08/05), plus enclosure notes and mailing envelopes
    • Billing statements, Patient billing statements, plus enclosure notes and mailing envelopes.
  • Creating and saving copies that are:
    • Personalized with your own professional practice information.
    • Patient Templates storing patient account information.
    • Insurance Claim Records storing specifically dated insurance claim information.

EZcms1500 Features…

  • Operations Bar for executing EZcms1500 operations.
