What is EZcms1500?

EZcms1500 is Queen City Computer Press, Inc.’s traditional enhancement of the CMS–1500 (formerly, HCFA-1500) form, the United States’ standard Health Insurance Claim Form that is used to expedite Medicare, Medicaid and private insurance company benefits.   • Insurance Claim Workspace — for preparing and printing CMS–1500 claim forms and setting up Insurance Claim Record copies of EZcms1500.
